You are purchasing a gift for: 'Jon Phillip Smith' Type in the initials in the following order: First Name Initial, LAST Name Initial, Middle Name Initial Example: JSP

When requesting a person's name, title, sorority: Type the name in the comment line or gift message line.

*We are not responsible for monogramming ordered in the wrong order, unless it is an error on our part, in which we will gladly send a replacement as soon as possible.
